Pathways to Home - Housing Program
Pathways to Home, funded through the Reaching Home initiative, is part of Canada’s national strategy to end homelessness. NCFC participates in Regina’s Coordinated Access program, where our housing team provides wraparound support to help remove barriers to safe, stable housing. Through intensive, team- based case management, we support individuals referred through this system by securing housing, addressing emergencies, and connecting clients to income support, employment services, and life skills development. Regular check-ins with clients and landlords, along with ongoing assessments to track progress and adjust supports, ensure steady movement toward long-term stability.
2024-2025 Highlights:
-45 families housed
-38 households graduated to independent living
-25 individuals received emergency supports to prevent eviction
-10 families connected to long term community supports
